We are seeking a full-time care coordinator to serve as part of our team on the Flourish Mobile Health Clinic. This person will provide trauma-informed, harm reduction-oriented care coordination services to vulnerable populations, including individuals affected by substance use disorders (SUD), homelessness, and limited access to healthcare. This position supports the clinic's interdisciplinary care model by providing care coordination specific to healthcare referrals and healthcare access, including but not limited to coordinating behavioral, primary care, and specialty health referrals. In addition, this person will provide crisis intervention, connect patients with community resources, engage in community outreach and coordinate on-site experiences for social work and public health students. Primary position responsibilities: Direct patient care support by assisting with providing screenings, brief interventions, and harm reduction to patients, in addition to maintaining accurate and confidential patient records and documentation. Care coordination through development of care plans, coordinating referrals and follow-up, and assisting with required application submissions for community resources to meet individual needs. Engage and build relationships with community partners to decrease barriers to care for patients. Coordinate on-site experiences for social work, public health or other undergraduate students. Respond to emergency situations which includes mental health, such as crisis intervention. The above statements describe the job’s essential responsibilities and requirements.
